Federal W-4 Form
Employee’s Withholding Certificate
Complete the Federal W-4 to tell us how much federal income tax should be withheld from your paycheck.
Complete the form online, print and sign it, then email us a scanned copy or clear photo of the signed form.
Oregon W-4 Form
Oregon Withholding Statement and Exemption Certificate
Complete the Oregon W-4 so we can calculate the appropriate Oregon state income tax withholding for your paycheck.

Form I-9
Employment Eligibility Verification
Federal law requires employees to complete Form I-9 to verify identity and authorization to work in the United States.
You will also need to provide acceptable identification documents as part of the verification process.
Complete the Form I-9 online, print and sign it and email us, then provide acceptable identification documents from the I-9 document list. Let us know if you have questions about document verification.
Authorization for Background Check
As part of the employment and placement process, Main Street Dental Staffing may obtain background information about you for employment-related purposes.
By signing this, you authorize Main Street Dental Staffing and its authorized background screening provider to obtain information about you, where permitted by law, from sources that may include courts, law enforcement agencies, corrections agencies, public records, and other appropriate sources.
